CMEchat 30: Behavior Change: Where Does CME fit?

Several new folks joined us as we discussed the intersection of the work of BJ Fogg and the role of CME in behavior change

Our topics were as follows:

#CMEchat homework: As we dive head long into behavior change, here is a great resource from @bjfogg bit.ly/PRUZA

T1: How do we know that the education that we provide to HCPs is impacting their behaviors? #CMEchat

T2: If we dig further, what are some types of behaviors we are trying to change? #CMEchat

T3: @bjfogg proposes that not all changes r equal, they differ in familiarity & duration. How can CME help? #CMEchat (http://bit.ly/PRUZA)

T4: If we simplify behavior further, how can CME impact a) motivation to change or b) one’s ability to change? #CMEchat

T5: If we can effect behavior change, what r our best practices in supporting changes overtime (span or path behaviors) #CMEchat

The transcript can be reviewed below or downloaded right here:

  • drpenzesjanos Interesting convo #CMEChat, thank you for the thoughts and links shared!
  • BrianSMcGowan @improvecme - exactly...teach them to see the impact of their changes. But, if you were wrong and change had no impact = #failing #CMEchat
  • improvecme #CMEchat Thanks again for a great episode of learning/opportunity for growth. Derek Dietze
  • DownstateCME Always!!RT @sd90mac6000: ...and team should include the PATIENT! #cmechat
  • spelletier RT @sd90mac6000: ...and team should include the PATIENT! #cmechat
  • sd90mac6000 ...and team should include the PATIENT! #CMEChat
  • dean_jenkins Communities have identity not peer pressure. RT @BrianSMcGowan: "Subjective norming" RT @theCMEguy: T5: Involvement in a CoP. #cmechat
  • CMEChat The real impact of the #CMEchat lies in your action - engage others to participate & please take action on things your have learned! G’day!
  • sd90mac6000 Absolutely. RT @spelletier: T5 team training also could play into sustaining change—hold each other accountable #cmechat
  • CMEChat As we get wrap up, please vote for the tweet of the day (#TOTD) seen during this week's #CMEchat.
  • CMEChat Thx for participating in #CMEchat. Discussions herein are assumed 2 b personal opinion & not that of current, past, or future employers.
  • dean_jenkins What she said! RT @spelletier: T5 team training also could play into sustaining change—hold each other accountable #cmechat
  • BrianSMcGowan "Subjective norming" RT @theCMEguy: T5: Involvement in a Community of Practice. Peer pressure for sustained behavior change #CMEchat
  • spelletier T5 team training also could play into sustaining change—hold each other accountable #cmechat
  • dean_jenkins Woot! RT @theCMEguy: T5: Involvement in a Community of Practice. #cmechat
  • improvecme T5 #CMEchat Example 2 sust chng, but don't know how long it lasts: help phys make connection between positive change & positive pt outcomes.
  • dean_jenkins T5 - CME should target 'culture' and 'clinical environment' to sustain behaviours. That can be challenging! #cmechat
  • BrianSMcGowan T5 - importantly to make span programs into path successes (this is what we do) we MUST engage the participant to find successes...#CMEchat
  • sd90mac6000 T5: t.co/j0ZQJ2r3 is an example of a project that changed behaviours, using CHE, patient ed'n, local champions, etc. #CMEChat
  • theCMEguy T5: Involvement in a Community of Practice. Peer pressure for sustained behavior change #CMEchat
  • DownstateCME pay for performance...rewarding them for verifiable outcomes RT @BrianSMcGowan: t5 - how do you sustain change #cmechat
  • spelletier What he said!! RT @dean_jenkins: T5 - getting the culture in a clinical setting right is most important for sustaining behaviours. #cmechat
  • BrianSMcGowan T5 - could it be, that if you are successful in the short-term changes, each successful behavior should feed/remind the next? #CMEchat
  • dean_jenkins T5 - getting the culture in a clinical setting right is most important for sustaining behaviours. #cmechat
  • spelletier T5 need reward system—tie into insurance (lower rates for better compliance)? What is possible on a system level? #cmechat
  • dean_jenkins @meducate Im just saying that sometimes a card is useful. Not for everything but for some purple spans. (correction!) #cmechat
  • BrianSMcGowan t5 - the problem with current strategies is they will wear off....Hawthorne proved this quite simply...how do you sustain change #CMEchat
  • dean_jenkins @meducate I'm just saying that sometimes a 'card' is useful. Not for everything but for some purple paths. #cmechat
  • theCMEguy Sure. Eventually reminders aren't needed GreenSpan -> BluePath RT @BrianSMcGowan: solution that is sustainable long term? #CMEchat
  • CMEChat T5: If we can effect behavior change, what r our best practices in supporting changes overtime (span or path behaviors) #CMEchat
  • dean_jenkins Nice systems approach. RT @sd90mac6000: Ottawa Model of Research Use - factors outside education. #CMEChat t.co/2vSfHsQh #cmechat
  • BrianSMcGowan @spelletier - AHRQ just launched that program - 10 Q you should ask your doc....important lever to behavior certainly! #cmechat
  • spelletier "T4 Automatic reminder system in EMR 2 use certain tools with certain patients?
  • #cmechat"
  • meducate @spelletier Or have the patients bring the tools #CMEChat
  • spelletier Thought: Include patients—educate pts 2 ask their docs 2 use tool. That'd annoy them into changing... #cmechat
  • improvecme T4 #CMEchat Another example: Model use of validated abbrev screening tools for depression have participants use it in CME activity w/case pt
  • meducate @dean_jenkins #CMEChat But they also have to remember that they have the cards in their pockets (or smartphones) or presence is useless
  • dean_jenkins The right card deck for certain Docs. RT @BrianSMcGowan: @improvecme @theCMEguy - & how many cards can a doc carry with them? #cmechat
  • BrianSMcGowan t4 - so far we have cognitive dissonance for motivation and checklists/visual reminders for ability...anything else/better/simpler? #CMEchat
  • dean_jenkins An example of a trigger for behaviour change. RT @improvecme: T4 Squeeze technique (chart) overcame complexity #cmechat
  • BrianSMcGowan @improvecme @theCMEguy - & how many cards can a doc carry with them? is this a solution that is sustainable long term? #CMEchat
  • dean_jenkins Fogg's Behaviour Model helps explain why a CME intervention works in one clinical setting but not another. #cmechat
  • improvecme T4 #CMEchat Squeeze technique overcame complexity barrier & simplified 1 of sev. prelim RA Dx techn. Major acceptance/use result in PCPs.
  • BrianSMcGowan @improvecme @theCMEguy - take the logical step from having the checklist or support tool - what is the trigger to use it. (B=MAT) #CMEchat
  • theCMEguy What @improveCME said :-) RT @BrianSMcGowan: .@theCMEguy - examples? #CMEchat
  • DownstateCME agree! BrianSMcGowan:T4-so if motivation is REALLY high, almost .....the rest of the time, simplicity is key! #cmechat
  • sd90mac6000 Food for thought: Ottawa Model of Research Use - many factors outside education. #CMEChat t.co/XPdzRkni
  • dean_jenkins T4 - It is the timing that is important for using the right trigger for a certain level of motivation and simplicity. (correction) #cmechat
  • BrianSMcGowan #TOTD. RT @dean_jenkins: @bjfogg 's analysis of different triggers - spark, facilitator, signal. t.co/KSrOMVxW #cmechat
  • improvecme T4 #CMEchat Simple tool example: brief pocket card/digital version of diagram showing how to do the "squeeze test" to help prelim Dx of RA.
  • dean_jenkins T4 - it is the timing that is important for using the right trigger for the right level of motivation and simplicity. #cmechat
  • BrianSMcGowan .@theCMEguy - examples? #CMEchat
  • theCMEguy T4: CME can impact ability to change by providing tools to overcome barriers to change #CMEchat
  • BrianSMcGowan T4 - so if motivation is REALLY high, almost nothing can stop behavior change...but the rest of the time, simplicity is key! #CMEchat
  • dean_jenkins I like @bjfogg 's analysis of different triggers - spark, facilitator, signal. t.co/IhzBFFGN #cmechat
  • drpenzesjanos "Change or die": gr8 piece ABT 3 empiric factors contributing to changing habits. t.co/6lgsBww6 #CMEChat
  • BrianSMcGowan +1. needed. RT @improvecme: T3 #CMEchat help folks see difference b/w current behavior & desired behavior, create cognitive dissonance.
  • sd90mac6000 @briansmcgowan If user has knowledge, but no translate into practice, then barriers at play. Facilitate overcoming those. #CMEChat
  • CMEChat T4: If we simplify behavior further, how can CME impact a) motivation to change OR b) one’s ability to change? #CMEchat (Fogg: B=MAT)
  • dean_jenkins RT @dean_jenkins: @spelletier motivation is one aspect of the @bjfogg model t.co/RJmfkKw9 #cmechat
  • theCMEguy T3: Simple tools for behavior change? How about reminders? #CMEchat
  • BrianSMcGowan @theCMEguy - no doubt me too...but we have data that nurses ignore alarms ("alarm fatigue")...there is a "high stakes" line here #CMEchat
  • dean_jenkins @spelletier motivation is one aspect of Fogg's model t.co/RJmfkKw9 #cmechat
  • spelletier @BrianSMcGowan yes! those intrinsic wishes—if we could tap into those, motivation prob is solved #cmechat
  • BrianSMcGowan Exactly, but what are these tools? RT @improvecme: T3 #CMEchat Then model behavior, provide simple tools to facilitate behavior.
  • dean_jenkins T3 - consultative work on changing behaviours "in situ" works but it is expensive. How to do it at a distance, or at a 'meeting'? #cmechat
  • improvecme T3 #cmechat As Sue suggests, commitments to change can help with this, do have relationship to actual behavior change.
  • spelletier This reminds me of a chat a few weeks ago—have to get back to the motivation. Why would they want to change? Have 2 crack that 1st #cmechat
  • BrianSMcGowan T3 - For any behavior change there are carrots & sticks...but these rarely mesh with intrinsic wishes. this is where CME should be. #CMEchat
  • dean_jenkins T3 - facilitating local discussion about behaviour triggers (as an external agency) is very challenging. #cmechat
  • sd90mac6000 Educate on the benefit of a system change - as simple as form adaptation in paper (urg!) chart. #CMEChat
  • theCMEguy .@BrianSMcGowan Maybe so. I know for myself I need to hear something multiple times before integrating it. I seek out repetition. #CMEchat
  • BrianSMcGowan @sd90mac6000 - are you over-simplifying the complexity of behaviour change. I worry about this alot, many gloss over this... #CMEChat
  • dean_jenkins Ha yes! RT @BrianSMcGowan: This is true too! RT @dean_jenkins: T3 - CME ... for a particular behaviour ... reduce its simplicity. #cmechat
  • improvecme T3 #CMEchat Then model behavior, provide simple tools to facilitate behavior.
  • rmtyner with gentle reminders along the way RT @spelletier: T3 use commitment to change as contracts to turn dots into echo behaviors? #cmechat
  • sd90mac6000 If you can can teach a system-level change, then behaviours can adapt to new system. #CMEChat
  • BrianSMcGowan Actually this is true too! RT @dean_jenkins: T3 - CME ... for a particular behaviour ... reduce its simplicity. #cmechat
  • spelletier T3 use commitment to change as contracts to turn dots into echo behaviors? #cmechat
  • improvecme T3 #CMEchat First help participants see difference between current behavior and desired behavior, create cognitive dissonance.
  • BrianSMcGowan .@theCMEguy - IMHO that wont work...we need 2 build a systems that makes reinforcement subtle, intelligent, and adaptive VS hammer #CMEchat
  • dean_jenkins T3 @theCMEguy recurrent triggers can be an annoyance though. #cmechat
  • EWC_SanJose_CA RT @spelletier: T3 CME changes in familiarity and duration too—match activity type/format/duration/followup to Fogg levels? #cmechat
  • improvecme T3 #CMEchat Breaking down more complex behaviors ie "optimal management of T2 diabetes" into specific components like check feet every visit
  • BrianSMcGowan This is brilliant! #TOTD RT @dean_jenkins: T3 - CME can facilitate the identification of local triggers for certain behaviours. #cmechat
  • BrianSMcGowan t3 - if we see perf. changes as type (start doing, do more, stop doing, or do less) & duration - what would CME look like #CMEchat
  • DownstateCME tough indeed..looking at root cause analysis is first step..RT@BrianSMcGowansource #cmechat
  • dean_jenkins Sorry increase simplicity! RT @dean_jenkins: T3 - CME ... for a particular behaviour ... reduce its simplicity. #cmechat
  • meducate T3 #CMEChat - if we aim to address familiar and/or relevant behavior it may be easier
  • theCMEguy T3: CME can be that constant pounding reinforcement (You need to do this! Do it! Do it!) that is needed to cause behavior change #CMEchat
  • drpenzesjanos RT @CMEChat: T3: Fogg suggests not all behavior changes r equal #CMEchat (http://t.co/gxHXkW9A) <= interesting approach,
  • dean_jenkins T3 - CME can facilitate the identification of local triggers for certain behaviours. #cmechat
  • spelletier T3 CME changes in familiarity and duration too—match activity type/format/duration/followup to Fogg levels? #cmechat
  • meducate @BrianSMcGowan #CMEChat - yes, for instance improving patient hand-offs, referrals, etc. can be the focus of the CME
  • dean_jenkins T3 - CME can increase the motivation for a particular behaviour and reduce its simplicity. #cmechat
  • BrianSMcGowan t3 - instead on 'communication' or 'Pt care' shouldn't we focus on the VERY specific decision making acts & support these. #CMEchat
  • CMEChat T3: Fogg suggests not all behavior changes r equal, they differ in familiarity & duration. How can CME help? #CMEchat (http://t.co/AxbnO7my)
  • BrianSMcGowan @DownstateCME - but what is the actual actionable behavior that would change??? it is tough to dig to the source... #cmechat
  • theCMEguy T2: Trying to go from DOT behavior to PATH behavior #CMEchat
  • dean_jenkins T2 - Checking out-of-office BP before making changes to hypertension treatment in X proportion of patients over next month. #weeds #cmechat
  • BrianSMcGowan T2 - i acknowledge topic 2 can lead to 30,000 ft answers, theoretical answers, & in the weeds answers...but we learn from each. #CMEchat
  • DownstateCME T@: reducing high risk occurences> better pt care #cmechat
  • ChargeAheadMktg RT @MGHAcademy: T2: what we're trying to change, ultimately, is their behavior re: delivering better Px outcomes via assessment, Tx, Rx, etc #cmechat
  • spelletier Why not? RT @BrianSMcGowan: T2 - could we suggest a behavior would be specific, measureable, actionable, realistic, AND time-bound? #cmechat
  • ChargeAheadMktg what he said —> RT @dean_jenkins: T2 We are trying to change the nature of the interaction with the patient. #cmechat
  • BrianSMcGowan T2 - could we suggest a behavior would be: specific, measureable, actionable (duh), realistic, AND time-bound? #CMEchat
  • MGHAcademy T2: what we're trying to change, ultimately, is their behavior re: delivering better Px outcomes via assessment, Tx, Rx, etc #cmechat
  • improvecme T2 #CMEchat We should be looking to change the incremental, elemental components of larger more complex behaviors w/reinforcement over time.
  • dean_jenkins T2 We are trying to change the nature of the interaction with the patient. #cmechat
  • BrianSMcGowan t2 - i would have answered this question quite differently a few weeks ago...but what would SMART objectives suggest? #CMEchat
  • spelletier T2 following guidelines/best practices in patient care for specific disease states #cmechat
  • BrianSMcGowan @improvecme - only if there's sufficient data to anchor the subjective & the surrogate back into reality - tougher than it sounds #CMEchat
  • theCMEguy Agree RT @BrianSMcGowan: most of what the CME has done w/ behavior change is based on surrogate marker or self report, no? #CMEchat
  • rmtyner T2 patient communication #CMEchat
  • sd90mac6000 T1 - Rx tracking possible in Canada, I believe. #CMEChat
  • spelletier @theCMEguy reinforcement is change—changing level of certainty in the behavior #cmechat
  • ChargeAheadMktg thank goodness for transcripts of #cmechat, cuz i seem to get bothered or pulled away every Wed! my Wed used to be so free. #damnit
  • CMEChat T2: If we dig further, what are some types of behaviors we are trying to change? #CMEchat
  • BrianSMcGowan t1 - most of what the CME community has done recently w/ behavior change is based on surrogate marker or self report, no? #CMEchat
  • theCMEguy Always "change"? Sometimes reinforcement RT @dean_jenkins: Education is behaviour change. Everything we do is a means to that end. #cmechat
  • improvecme T1 #CMEchat We can know CME is changing their behaviors by subjective, self-reported means, and through objective measures like EMR, claims.
  • spelletier Do any object as being too Big Brotherish? RT @theCMEguy: T1: On a higher lever, could use EMR, medical claims data. #cmechat
  • DownstateCME difficult 2 do.RT@meducate:@BrianSMcGowan #CMEChatT1 Measuring/tracking r often synonymous..need 2 have a strategy&involve learners #cmechat
  • sd90mac6000 If systems-level measurement is possible. MT @briansmcgowan: T1 - are there ways where changes can be captured w/o survey/intrude? #CMEchat
  • ChargeAheadMktg RT @theCMEguy: Less intrusive, too RT @theCMEguy: T1: On a higher lever, could use EMR, medical claims data to track behavior change. #cmechat
  • dean_jenkins Data definitely key. RT @theCMEguy: T1: On a higher lever, could use EMR, medical claims data to track behavior change. #cmechat
  • theCMEguy Less intrusive, too RT @theCMEguy: T1: On a higher lever, could use EMR, medical claims data to track behavior change. #cmechat
  • dean_jenkins Education is behaviour change. Everything we do is a means to that end. #cmechat
  • BrianSMcGowan .@MGHAcademy - now that is the ultimate win win.... #cmechat
  • theCMEguy T1: On a higher lever, could use EMR, medical claims data to track behavior change. #cmechat
  • MGHAcademy @briansmcgowan we try to involve "them" in terms of sharing aggregate data, so it feels there is a value exchange for their time #cmechat
  • spelletier And does "them" know they need to change? RT @BrianSMcGowan: but them gets tired of being bothered. #cmechat
  • BrianSMcGowan T1 - are there ways where the changes happen and can be captured w/o having to survey or intrude in the workstream? #CMEchat
  • meducate @BrianSMcGowan #CMEChat T1 Measuring and tracking are often synonymous. We simply need to have a strategy and involve the learners
  • spelletier ;>RT @sd90mac6000: Agreed! Behaviour change is difficult! RT @dean_jenkins: I just cant type behavior (grr) goes against the grain. #cmechat
  • meducate Hello my friend! RT @drpenzesjanos: Janos from cludy Budapest lurking #CMEChat
