#CMEchat 59: How does 'motivation' affect our educational planning?

Perhaps it can be attributed to the alignment of our collective New Year's resolutions, but the CMEchat experience has really amped up so far this year. Today's chat was among the best ever!

Today our topics were as follows:

(1 of 2) We talk about behavior change a lot w/i CME, but w/o motivation there can be no behavior change... #CMEchat

(2 of 2) …today we’ll explore how motivation affects behavior change “behind the scenes” – looking at motivation w/i our planning #CMEchat

T1 In general, what do you feel fuels motivation? And, how comfortable are you with the idea of “affecting” one’s motivations? #CMEchat

T2 In general, how does ‘motivation’ impact your CME planning efforts? Think: about staff, faculty, or learner motivation? #CMEchat

T3 How might you motivate FACULTY to become better facilitators? How do you deal with unmotivated faculty? #cmechat

T4 How might you motivate LEARNERS to become more engaged learners? How do you deal with unmotivated learners? #cmechat

BrianSMcGowan              Good morning to the #CMEchat-ters. Very motivated to have this conversation this morning. How about you?

theCMEguy        @BrianSMcGowan I see what you dd there. #cmechat

BrianSMcGowan              Brian here in foggy but mild southeastern pennsylvania. #CMEchat.

BrianSMcGowan              @theCMEguy ;-) #cmechat

Savvy_CME        Pam from oddly warm DC metro area, focused on compliance and accreditation but interested in behavior change (not just my own) #CMEchat

theCMEguy        Derek in dreary Chestnut Hill. At least it's not freezing. #CMEchat

meducate           #cmechat Lawrence here in increasingly sunny Long Island before I drive to foggy Pennsylvania

BrianSMcGowan              @Savvy_CME - long time, no see. Welcome #CMEchat

rmtyner               Michelle from frigid Indiana...  #cmechat

theCMEguy        @meducate Hey, don't blame us #CMEchat

triciawil @BrianSMcGowan Good morning!  Tricia here, in rainy Charlottesville... #cmechat

improvecme      #cmechat Derek in freezing Arizona

BrianSMcGowan              Looks like everyone is gathering nicely...welcome to Derek and lawrence and tricia, Michells, Derek (2), and Haley! #CMEchat

CMEChat             (1 of 2) We talk about behavior change a lot w/i CME, but w/o motivation there can be no behavior change... #CMEchat

CMEChat             (2 of 2) …today we’ll explore how motivation affects behavior change “behind the scenes” – looking at motivation w/i our planning #CMEchat

theCMEguy        Really? What 's freezing in AZ? 60? :-) RT @improvecme: #cmechat Derek in freezing Arizona

BrianSMcGowan              AZ if? RT @theCMEguy: Really? What 's freezing in AZ? 60? :-) RT @improvecme: #cmechat Derek in freezing Arizona

meducate           @theCMEguy @improvecme #cmechat Yes, but it is a DRY cool

CMEChat             T1 In general, what do you feel fuels motivation? And, how comfortable are you with the idea of “affecting” one’s motivations? #CMEchat

improvecme      #cmechat In AZ in the 20s past 3 nights. Loosing our citrus.

ArdgillanGroup Mornin' all, Karen here from foggy Philly! #CMEchat

theCMEguy        Wow! Really? Yikes... RT @improvecme: #cmechat In AZ in the 20s past 3 nights. Loosing our citrus.

BrianSMcGowan              t1 - I am a student of @bjfogg and @DanielPink - and I like to think about how motivation impacts learning...#CMEchat

meducate           RT @meducate: T1 Motivation comes from comfort "I agree with that" or discomfort/cognitive dissonance "That's not me" #cmechat

rmtyner               T1 considering motivation of target audience is critical when developing a needs assessment #cmechat

improvecme      T1 Motivation to learn fueled by many different things, but desired to solve a problem/challenge an important one. #cmechat

theCMEguy        T1: Seems hard to generalize motivation. Aren't we all motivated by different things? #CMEchat

Savvy_CME        T1: I think nowadays, what fuels motivation is the lack of time barrier- efficiency is ruling. #CMEchat

meducate           T1 #cmechat You also have to know your audience...we fail at this in CME in many cases

BrianSMcGowan              t1: Motivation comes in many flavors...but it seems most critical to this discussion is 'intrinsic' vs 'extrinsic' forms #CMEchat

improvecme      T Perhaps we can affect motivations by teaching learners to think about how their motivations affect their learning & performance. #cmechat

triciawil @theCMEguy It depends on what the goal is... #cmechat

nlafferty              Hello #cmechat from Dundee, Scotland, I suspect I will be mostly lurking on this week's chat

CMEChat             .@theCMEguy - sure, but like what? let the ideas flow! #CMEchat

Savvy_CME        T1: If it improves patient care (and not just the bottom line) I don't see a problem with affecting motivation, creating incentives #CMEchat

theCMEguy        T1: In CME, many motivated by: "I have a problem, how can I solve it?" #cmechat

meducate           @nlafferty Glad that you made it to #cmechat, Natalie!  Dundee is the location of some good #meded

BrianSMcGowan              #inception RT @improvecme: T Perhaps we can affect motivations by teaching learners to think about how their motivations... #cmechat

nlafferty              " @meducate: @nlafferty Glad that you made it to #cmechat, Natalie!  Dundee is the location of some good #meded" Indeed :)

Savvy_CME        T1: Perhaps motivation should be the first question in a pre-test, before "do you know where a spleen is?" and other softballs #CMEchat

meducate           T1 #cmechat Learners need a reason to be motivated

BrianSMcGowan              t1: when I origincally wrote this topic, i thought of using the word 'manipulating' one's motivation...is that fair? #CMEchat

theCMEguy        T1: and some are motivated by factors such as "uh oh...I need to get 5 more credits to meet requirements..." #cmechat #honesty

BrianSMcGowan              huh? RT @meducate: T1 #cmechat Learners need a reason to be motivated

Savvy_CME        @BrianSMcGowan Leading a horse to water, not making them drink... #CMEchat

meducate           T1 #cmechat Manipulation connotates something bad (or chiropractic).  It isn't manipulation that we are doing, it's encouraging

theCMEguy        @BrianSMcGowan Sure. This is why we offer incentives for surveys, etc... #cmechat

BrianSMcGowan              T1: fear is a great motivator, no? seems that there is a slippery slope b/w causing cognitive dissonance and fear... #CMEchat

Savvy_CME        @theCMEguy And some are motivated by factors of "I'm losing money, I want to learn how to make more" #CMEChat #overlyhonestCME

triciawil T1 #cmechat I'd like to think the motivation is self-improvement...

rmtyner               T1 too much fear could result in no action at all  #cmechat

improvecme      T1 Some other motivations: I need credit, I have competition/want 2 b better than them, I want 2 provide better care for X patients #cmechat

meducate           @BrianSMcGowan T1 #cmechat I mentioned that earlier - either comfort or discomfort can motivate

BrianSMcGowan              Mastery is a great fuel for motivation. agreed! RT @triciawil: T1 #cmechat I'd like to think the motivation is self-improvement...

BrianSMcGowan              same w/ too much dissonance, no? RT @rmtyner: T1 too much fear could result in no action at all  #cmechat

Savvy_CME        .@BrianSMcGowan What are they afraid of?  Getting sued? #CMEchat

BrianSMcGowan              i see that is a pretty big stick, but wld you ever use it? RT @Savvy_CME: .@BrianSMcGowan What are they afraid of?  Getting sued? #CMEchat

theCMEguy        Ideally it is. Not always the case, tho RT @triciawil: T1 #cmechat I'd like to think the motivation is self-improvement...

meducate           Well, you have exposed yourself!  Welcome! RT @carvicab: Lurking #CMEchat

BrianSMcGowan              T1: @DanielPink  talks about intrinsic motivations being MUCH more important: Autonomy, Mastery, Purpose - key in medicine. #CMEchat

CMEChat             T2 In general, how does ‘motivation’ impact your CME planning efforts? Think: about staff, faculty, or learner motivation? #CMEchat

triciawil Agreed RT @theCMEguy: Ideally it is. Not always the case, tho RT @triciawil: T1 #cmechat

improvecme      T1 Another motivation: save money (i.e., reduce malpractice insurance costs) by taking specific CME. #cmechat

nlafferty              @carvicab You're not alone! #cmechat

theCMEguy        . @Savvy_CME made a good point before: if the bottom line is improved patient care, does the root of motivation matter? #cmechat

CMEChat             t1: i wld also think Autonomy, Mastery, Purpose - would be key in learning too! Might I suggest Drive t.co/HLkwfKlE #CMEchat

ArdgillanGroup Fear of litigation  - issue on Pain management #REMS #CMEchat  Education on screening and documentation can be the carrot.

theCMEguy        @HaleyMarie3 Agreed and I have no problem with that being the motivation... #cmechat

BrianSMcGowan              @HaleyMarie3 - makes you consider whether altruism is a motivator in medicine...or is it always #WIIFM? #cmechat

Savvy_CME        T2: Motivation- better content (not just recycled stuff), engaging faculty, improved formats, interactivity (ARS) = learner :)  #CMEchat

theCMEguy        @BrianSMcGowan @HaleyMarie3 I would think it's a combo of both for many, no? #cmechat

BrianSMcGowan              I think when you are dealing with life or death behaviors (like medicine) the idea of motivation in learning can be very delicate! #CMEchat

ArdgillanGroup Feedback!  #WIIFM?  #cmechat  personalized reports?

rmtyner               T2 IMO the time constraints challenge the altruism - even more important to consider motivation #cmechat

triciawil @CMEChat Thanks for the suggestion - will download it! #cmechat

Motivology         rt @rmtyner2013T2 IMO the time constraints challenge the altruism - even more important to consider motivation #cmechat  #motivology

BrianSMcGowan              t2: we 'want' learners to sit in the front of the room...how do we motivate this behavior? #CMEchat #startsimple...

BrianSMcGowan              t2: we want learners to engage in small group breakouts and not leave the room, how do we motivate that? @theCMEguy #CMEchat

Savvy_CME        .@BrianSMcGowan keep all the plugs and outlets at the front of the room...maybe some comfy chairs. #CMEchat

ArdgillanGroup Ask learners to declare their goals ? #cmechat

rmtyner               T2 i think that engaging the learners comes from the content - not the same old presentation - and interactive faculty #cmechat

BrianSMcGowan              t2 - @rmtyner shared an old Fox paper that explored perceived vs actual needs fueling motivation. #heartofmotivation #CMEchat

improvecme      T2 What's the value in motivating the behavior to sit at front of the room if the CME is not designed to be interactive/engaging? #cmechat

BrianSMcGowan              t2 - we have questioned learners self assessment for years, but their flawed assessment is their perception #truth #motivation #CMEchat

BrianSMcGowan              @improvecme - touche! #cmechat

rmtyner               +1 MT @improvecme: T2 Whats value motivating behavior to sit at front of room if CME is not designed to be interactive/engaging? #cmechat

BrianSMcGowan              t2: but we want faculty to engage vs lecture, how do we leverage motivation to drive this behavior? #CMEchat

triciawil T2 #cmechat @BrianSMcGowan @theCMEguy IMO small group breakouts involve a balancing act to engage introverts & extroverts

CMEChat             Lets get a bit more specific with T3...#CMEchat

elearningCME    t2: I'd rather have audience interact with peers than me depending on size of audience. Front of room overrated. #cmechat

theCMEguy        @BrianSMcGowan Explain, encourage, and understand. You can't motivate someone to do someting they really don't like. #cmechat

BrianSMcGowan              @elearningCME - i'd rather not interact with you either ;-) #cmechat

rmtyner               T2 assessment should be more quantitative - with right & wrong answers - if i only got 3 out of 10 right maybe something is wrong?? #cmechat

elearningCME    Touche! RT @BrianSMcGowan: @elearningCME - i'd rather not interact with you either ;-) #cmechat

BrianSMcGowan              @theCMEguy - but isn't that the heart of the question. are you sure you can't? I think you can, but should you and when? #cmechat

CMEChat             T3 How might you motivate FACULTY to become better facilitators? How do you deal with unmotivated faculty? #cmechat

theCMEguy        Yes RT @elearningCME: t2: I'd rather have audience interact w/ peers than me depending on size of audience. Front of room overrated #cmechat

improvecme      T2 Take the opportunity to tell learners about adult learning and how to optimize it. What evidence shows aids their learning. #cmechat

BrianSMcGowan              @improvecme - think there is something to the adage of showing them vs telling them, no? #cmechat

BrianSMcGowan              t3: what motivates faculty to accept speaking engagements in the first place? Critical first piece of the puzzle, no? #CMEchat

elearningCME    T3: Unmotivated faculty...take them out back and.... #cmechat

rmtyner               T3 creating a faculty that is interested in teaching is the first step in motivating the learner #cmechat

meducate           Go Brooklyn on them! RT @elearningCME: T3: Unmotivated faculty...take them out back and.... #cmechat

BrianSMcGowan              t3: If you want to teach, we can help you teach better! If you want the prestige/fame, then ba-bye. #CMEchat

rmtyner               RT @elearningCME: T3: Unmotivated faculty...take them out back and.... #cmechat

theCMEguy        T3: Have found sharing eval results with faculty can sometimes be a motivator, particularly when comments are harsh... #cmechat

ArdgillanGroup T3 motivate faculty by engaging them every step - planning, thro assessment and report/pub, not  "will you give your xx talk" #CMEchat

BrianSMcGowan              #TOTD RT @ArdgillanGroup: T3 motivate faculty by engaging them every step...not  "will you give your xx talk" #CMEchat

Savvy_CME        T3: Motivate faculty with learner feedback, find a physician advocate/leader for unmotivated faculty- often it takes peer-to-peer #CMEchat

meducate           T3 #cmechat Faculty need to behave as #goodeducators and be a part of the process

DrPlumEU           RT @rmtyner: RT @elearningCME: T3: Unmotivated faculty...take them out back and.... #cmechat

BrianSMcGowan              t3: Has anyone ever asked the faculty, what motivates you to travel so much, to 'lecture' so much? Any guesses? #CMEchat

elearningCME    T3: Good suggestions but if they don't work? Are you ok with that? If you're not okay, don't use that faculty. #cmechat

Savvy_CME        T3: Take the "who cares" info out of evaluation results to send to faculty- beyond just hot/cold room. Tease out what learners want #CMEchat

theCMEguy        T3: a strong program director can make a difference with unmotivated faculty. Sometimes they need a peer to give kick in the *ahem* #cmechat

CMEChat             T1-T3: (you guys and gals are knocking the cover off the ball!) #CMEchat

improvecme      T3 Old method-contract faculty who give canned presentation. New method-engage faculty in uncovering gaps/ask them 2 help fill gaps #cmechat

elearningCME    Prestige. Attention. RT @BrianSMcGowan: t3: Has anyone asked the faculty, what motivates you to traveland 'lecture'? Any guesses? #CMEchat

DrPlumEU           @meducate #cmechat Am GP in UK who teaches medical students.  Overcoming isolation of community teachers is important How to do this well?

ArdgillanGroup @BrianSMcGowan re:travel. Reasons vary from $ to career development to delight in interaction and teaching #CMEchat

theCMEguy        @BrianSMcGowan Are we dancing around the $$$ issue? #CMEchat #softshoe

BrianSMcGowan              T3: Ask you faculty b4 confirming: Are there good motivations, can you use them? Are there bad motivations, can you replace them? #CMEchat

meducate           @DrPlumEU #cmechat There are several strategies for that - collaborative teaching, team up with visiting professors, provide ed tools

elearningCME    t3 - My experience is that organizers are afraid to confront the low performers...and that's on us as educators...to try harder. #cmechat

DrPlumEU           @ArdgillanGroup @BrianSMcGowan As GP tutor it isn't money/career; it's pleasure of teaching which is different from usual clinic #cmechat

BrianSMcGowan              @theCMEguy - I dont think so... i turn down honorarium far more often than I accept it. Money is not worth the hassles. #CMEchat #time

Savvy_CME        T3: Need leadership to step in: if a faculty member is dull, someone needs to light a fire. #CMEchat

ArdgillanGroup @theCMEguyNot sure $$ as big as before - losing $$ by leaving practice, honoraria capped, institution rules, transparency #CMEchat

DrPlumEU           @meducate #cmechat best motivation for me is end of block assessment meetings with med school faculty/physicians. Am only GP who does this

BrianSMcGowan              t3: as we move to #P4p then the money motivators will be derivative at best...salaried is salaried. #CMEchat

meducate           @DrPlumEU #cmechat That is good for you but scary that no other GPs do this

theCMEguy        @ArdgillanGroup True, and some can't accept $ for speaking or it goes directly to dept. #CMEchat

CMEChat             Almost time for the final topic.... #CMEchat

Savvy_CME        T3: Content should be reviewed for more than just content, bias, how about redundancy? Ask if they've ever done presentation before #CMEchat

DrPlumEU           @theCMEguy @ArdgillanGroup #cmechat Any money earned in my activities (or those of partners here) goes directly into practice income

theCMEguy        T3: Hey, maybe some faculty just like to teach others and THAT'S their motivation... #CMEchat

improvecme      T3 Some suggested measures of faculty effectiveness: knowledge, competence & performance change in CME part. Feed back 2 faculty. #cmechat

BrianSMcGowan              @DrPlumEU - ever think about where that 'pleasure' is rooted? Need more info is this is to be 'scalable'... #cmechat

theCMEguy        @DrPlumEU @ArdgillanGroup Yes, this seems to be more and more common #CMEchat

BrianSMcGowan              @theCMEguy - IMO, that is not specific enough to 'use' ...need to understand the root of the motivation... #CMEchat

DrPlumEU           @Savvy_CME #cmechat In community teaching very hard to be prescriptive - controlling what is seen enormously difficult in general clinic

CMEChat             T4 How might you motivate LEARNERS to become more engaged learners? How do you deal with unmotivated learners? #cmechat

BrianSMcGowan              T4 - to understand this question you have to accept that 'there is no learning without motivation...' #CMEchat #deathtaxes&motivation

theCMEguy        @BrianSMcGowan Disagree. Root of motivation is that they like to help people. That is the motivator for some. #CMEchat

elearningCME    T4 - Important to determine why they are unmotivated. Is it not what they expected? Is the learning compulsory? ??'s #cmechat

DrPlumEU           @CMEChat #cmechat T4 direct observed skills like examination/hx taking with debrief using Pendleton Rules involving all the students

BrianSMcGowan              t4: the general assumption therefore is that the needs assessment process is identifying a shared motivation to learn, no? #CMEchat

Savvy_CME        T4: Games sometimes help with interaction- whether through ARS or Jeopardy. #CMEchat

improvecme      T4 Can't force them but take some time at beginning of CME 2 explain why/how & what kind of engagement will enhance their learning. #cmechat

BrianSMcGowan              @theCMEguy - humbly disagree...the root would then be they feel 'purpose' or they feel 'prestige' #underlyingfeelings #CMEchat

DrPlumEU           @BrianSMcGowan #cmechat Student feedback at end of year is motivating. My students look forward to coming to my practice; value what we do

ArdgillanGroup Not done enough! RT @improvecme: take time at beginning of CME 2 explain why/how & what kind of engagement will enhance learning. #cmechat

improvecme      T4 Physicians may not think a lot about how they learn/translate it into practice. Good 2 discuss it with them/ask 2 reflect on it #cmechat

BrianSMcGowan              t4: At least in the US Docs are supreme test takers...many love being tested vs loving learning, agree? #CMEchat #motivation

theCMEguy        T4: Potential to increase engagement by offering multiple avenues of engagement (open mic vs monitored Twitter stream) #cmechat

Savvy_CME        T4- I've used "intent to change" worksheets- "what do you hope to change as a result of this activity?"- get them thinking. #CMEchat

BrianSMcGowan              @improvecme - could this be done at 'scale'? #cmechat

DrPlumEU           @improvecme T4 #cmechat Learning who the student is, where she/he comes from can overcome inertia. Hospital drs often fail to do this

BrianSMcGowan              #TOTD RT @Savvy_CME: T4- [ask them] "what do you hope to change as a result of this activity?"- get them thinking. #CMEchat

CMEChat             Thanks for participating in #CMEchat. Discussions are assumed to be personal opinion & not that of employers

CMEChat             As we wrap up, please vote for the tweet of the day (#TOTD) seen during this week's #CMEchat – RT your favorite comment!

DrPlumEU           #cmechat T4- I've used "intent to change" worksheets- "what do you hope to change as a result of this activity?"- get them thinking. #TOTD

theCMEguy        @BrianSMcGowan humbly disagree w/ your humble disagreement. Possible to "help" someone without underlying motivation IMO #CMEchat

BrianSMcGowan              @Savvy_CME - but what if what they want to learn is NOT what I want to teach. They lose, I win. #sadreality #CMEchat

ArdgillanGroup TOTD Yes - get practical!RT @improvecme: T4 Phys may not think about how to translate into practice. Good 2 discuss it or  reflect #cmechat

BrianSMcGowan              Thanks to all of you for coming out today for #CMEchat. Might be our highest TPM, but clearly one of our best discussion!
